On September 1, the informal care square will open its proverbial doors at Amphia Hospital. Here, a team of care and welfare workers from Surplus and Thebe will prepare patients and their caregivers for their discharge from the hospital. For example, with instruction on the drip glasses or support stockings, but also with information on other support, such as the WMO budget and opportunities to strengthen/increase one's own network.
Why a informal care plaza?
Now patients often lie in hospital unnecessarily long because district nursing is not available. Or vulnerable elderly people are admitted to the hospital without medical treatment indication. With the project ‘working together in the chain’, of which the informal care square is a part, Surplus, Thebe and Amphia want to ensure that there is a better flow in the chain and improve cooperation between the professionals of the various organizations. With the Mantelzorgplein we want to ensure that patients require less professional aftercare at home by making the patient and their caregiver(s) self-reliant. Here welfare and prevention also have an important role to play in preventing hospitalization wherever possible.
The informal care plaza starts small with the eye clinic, Internal Medicine and Geriatrics.
